Appointments

We know you have many choices when choosing a dermatologist in the WNY area so we have made requesting an appointment a simple and convenient process. If, for any reason, you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible. If appointments are canceled less than 24 hours, there will be a $25 charge for any missed appointments and a $100 charge for procedure appointments.

Please have the following when you come in for your appointment:

2.  Photo ID
3.  Copayment/Deductible payment
4.  Medication List
5.  Pharmacy Information 
6.  Insurance Referral information if required by your plan

Insurance and Billing

We accept most traditional insurance plans. Please contact our office to verify the acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.

Payment Options

We accept checks, cash, or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details.

Lab Services

Please note regarding office biopsies, Niagara Dermatology Associates, LLC sends all biopsies to the University of Rochester Medicine Labs Strong Memorial Hospital. 

Please remember that YOU are responsible for ensuring that our office has the following information up to date:

1. - Insurance coverage - PLEASE PRESENT YOUR CARD AT EVERY VISIT
2. - Mailing/billing address
3. - Phone number
4. - Preferred pharmacy
5. - Any changes in medications, allergies, surgeries, or other medical history updates
